Complete this form to join our affiliate marketing group.  All information requested is required in order to participate.  Commissions are paid out 2 months following the sale.  For example, if a customer purchases a product or service on 12/01/06 then the commission will be paid in the March payment.  The advertiser decides the amount of the commission.  Checks are mailed out on the first day of the month.
